Dear Minister Fukuoka,
We are writing on behalf of the Contraception Committee of the International Federation of
Gynecology and Obstetrics (FIGO), a global organization composed of professional societies of
obstetricians and gynecologists worldwide. FIGO’s mission is to improve the health and rights of
women and newborns, reduce healthcare disparities, and advance obstetric and gynecological
practice worldwide.
We commend the efforts of your Ministry to explore new approaches to improve access to
emergency contraception (EC) in Japan. We would like to offer technical input based on
international evidence and rights-based standards, which we hope will support informed decisionmaking regarding EC availability.
FIGO’s guidance, developed in 2018 in collaboration with the International Consortium for
Emergency Contraception (ICEC) (and available in Japanese) is clear:
•
EC pills are safe and self-diagnosable. No medical screening, exams, or tests are needed
before use.
•
They are appropriate for over-the-counter (OTC) access, as endorsed in most countries.
•
No routine follow-up is required unless the user seeks further support.

Further, the available evidence highlights that EC pills act by preventing ovulation and are most
effective when taken promptly. Regulatory barriers such as prescription requirements and age
restrictions can delay access and reduce effectiveness, increasing the risk of unintended
pregnancies.
Levonorgestrel (LNG) EC pills meet all international criteria for OTC status. They are:
•
Safe, with no potential for overdose or addiction
•
Well-tolerated, even with repeated use
•
Safer than many OTC medications, such as aspirin
